Vermont requires a bill of sale to title or register a vehicle. Notarization is not required for a Vermont vehicle bill of sale. Vermont publishes an official form (Bill of Sale and Odometer Disclosure Statement (Form VT-005)).
- Vermont's official form is the Bill of Sale and Odometer Disclosure Statement, Form VT-005, published by the Vermont DMV; it is a combined bill-of-sale + odometer form usable for both vehicles and vessels (VIN/HIN field).
- No notarization is required for a Vermont vehicle bill of sale; the VT-005 form provides only buyer and seller signature lines (no notary block). Notarization only arises separately on the Affidavit of Non-Titled Vehicle (Form VT-025) when registering an older non-titled vehicle.
- A signed bill of sale is required when registering/titling: it establishes the chain of ownership and is used for Purchase and Use Tax purposes. For vehicles 15 or more model years old, Vermont is a non-titling jurisdiction and issues a registration in lieu of a title; the bill of sale plus prior registration documents ownership.
- The VT-005 must include the year, make, model, VIN/HIN, odometer reading, purchase price, sale date, and the signatures of both seller and buyer.

Federal law (49 U.S.C. sec. 32709; 49 CFR Part 580) and Vermont law require a written odometer disclosure on transfer of ownership. Per the 2021 NHTSA rule (effective Jan. 1, 2021) reflected on Form VT-005, vehicles model year 2011 and newer are subject to odometer disclosure for 20 years (a 2011 vehicle stays subject until 2031, 2012 until 2032, etc.), replacing the old 10-year/pre-2011 exemption. Vermont requires the Odometer Disclosure Statement for self-propelled vehicles model year 2011 or newer with a registered weight of 16,000 lbs. or less; both buyer and seller must sign.
