4. Is it true that you have never been ordered by a court to be detained in a hospital or secure facility because of your mental condition, on a criminal charge?

5. Is it true that you have never been indefinitely disqualified from driving by a court?

6. Have you paid in full any fine, reparation, or court costs you were ordered to pay?
