New York requires a bill of sale to title or register a motorcycle. Notarization is not required for a New York motorcycle bill of sale. New York publishes an official form (Form MV-912 (Vehicle Bill of Sale)).
Federal law (49 CFR 580) requires a written odometer disclosure at transfer for vehicles under 20 model years old. New York implements this on the certificate of title: the NY DMV states that for motor vehicles manufactured in or after the 2011 model year that are 20 model years old or newer, the seller must complete the odometer disclosure statement on the back of the title certificate; odometer disclosure is not required for older vehicles. Note: the MV-912 bill of sale form itself does NOT contain an odometer field (only year/make/model, VIN/HIN, price, signatures, and a free-text terms box) -- odometer disclosure is made on the title.
